The Care Manager, Telephonic Nurse 1, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 1 work assignments are often straightforward and of moderate complexity.

The Care Manager, Telephonic Nurse 1 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. Understands own work area professional concepts/standards, regulations, strategies and operating standards. Makes decisions regarding own work approach/priorities, and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.

Required Qualifications

  • Must reside in Puerto Rico
  • Must be fully bilingual (English/Spanish)
  • 2+ years of relevant experience
  • Licensed Practical Nurse (LPN) in Puerto Rico or equivalent with no disciplinary action.
  • Comprehensive knowledge of Microsoft Office applications, including Word, Excel, and Outlook.
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences

Preferred Qualifications

  • Prior experience with Medicare.
  • Previous experience with electronic case note documentation and experience with documenting in multiple computer applications/systems.
  • Experience working with geriatric population.
  • Experience with health promotion, coaching and wellness.
  • Knowledge of community health and social service agencies and additional community resources.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
